This is first for those going to Ft. Dodge and secondly for everyone else...I need the health forms on the blog filled out immediately along with the permission slip signed so I can take kids to Ft. Dodge....they are listed under parent resources...there are two health forms and a permission slip...I need the permission slip signed and dated---keep the bottom half for your records. Write in Ft. Dodge as the tournament please. Your students can give me the forms before we leave on Saturday!
I will need the health forms for everyone so please--please-please get it done! The concept was that I don't have to time chase everyone down for the forms so we put it on the blog but apparently everyone at the meeting forgot about them.
